Thanks to Marcus for the comment pointing to this post, showing how to disable the Insert key in Word. It's a nice workaround, if the app you're using allows you to assign a macro to the Insert key, as Word does.

I tried the same thing in VS 2005, but it doesn't recognize the Insert key in the “shortcut key“ edit box. I wonder if this is just a UI limitation? Running FILEMON while saving a key assignment in VS 2005 shows it writing to two files in the profile: Current.VSK and User.VSK, but the VSK files are binary (bummer, here I thought VS was moving all its config to XML).

I've started a wiki page so folks can post workarounds like this to share.

Why not use the Scan Code Mapper, which is built into Win2K+? You can just reassign INS to do nothing and some out-of-the-way key to be your INS. The syntax is cryptic and the UI tool seems to have disappeared from the Microsoft site. But a cryptic API has never stopped you before? :) Just a thought.
